Death Certificate Information


The Anatomy Bequest Program files the death certificate with the state of Minnesota, however, we are unable to provide any copies of the filed death certificate.

To obtain certified copies of the death certificate please contact the registrar's office in your area. It is no longer necessary to go to the registrar's office of the county where the death occurred. The certified copies of the death certificate are generally available within three weeks of the death. Rarely are they available before that period of time.

It is advisable to check the status of the death certificate before attempting to obtain a copy at the registrar's office. You may check the status of the death certificate by contacting the Registrars office in your area or by contacting the Anatomy Bequest Program at 612-625-1111 or bequest@umn.edu.

DEATH CERTIFICATE APPLICATION

A death certificate application must be completed regardless if the request is in person or via mail or fax.

DEATH CERTIFICATE CHARGES

$13.00 for the 1st copy and $6.00 per each additional copy ordered at that time.

Note: Orders by fax or mail that will be charged to a credit card will have an additional $5.00 fee for processing.
